CONSENSO INTERNACIONAL
ADHD Y ABUSOS EN LA PRESCRIPCIÓN DE PSICOFÁRMACOS A MENORES
(Italia, enero de 2005)
"Arriba las manos de los niños" es la campaña más visible de lucha por la defensa del Derecho a la Salud de los Niños organizada en Italia y es en efecto, la que actualmente se expande con mayor rapidez por el resto de Europa a través de iniciativas semejantes.
El objetivo de ésta campaña es brindar una correcta y completa información a la población (docentes, padres, adolescentes etc.) sobre el tema del abuso en el suministro de psicofármacos a niños y adolescentes que, con casi once millones de chicos crónicamente dependientes de estimulantes semejantes a las anfetaminas en los Estados Unidos, constituye una auténtica emergencia sanitaria que se ha importado también a Italia (para mayor información dirigirse a nuestro sitio www.giulemanidabambini.org)
La iniciativa, gestada por la red de trabajadores voluntarios de hospitales, que se reunió en un Comité de Asociaciones y Entes representativos de más de 6 millones de italianos, esta patrocinada por la RAI -Radio y Televisión Italiana-. Han adherido a la Campaña rostros famosos del mundo del espectáculo y de la ciencia, desde Ray Charles a Beppe Grillo entre otros. La Campaña cuenta con el aporte de un calificado Comité Científico que ha venido realizando una serie de actividades articuladas de divulgación en todo el territorio.
La iniciativa no tiene fines de lucro; es a-partidaria y no confesional. El portal informativo en Internet, realizado por los voluntarios, constituye actualmente el más rico y articulado disponible en Italia, sobre estos temas.
El síndrome denominado "Disturbios de atención con/sin hiperactividad" (ADHD) ha sido objeto de una cantidad impresionante de investigaciones experimentales y de estudios científicos. Los tratamientos que se deciden hoy en día son fundamentalmente de carácter farmacológico (estimulantes del tipo anfetamínico entre otros) y en medida menos significativa, intervenciones psicoterapéuticas o pedagógicas orientadas al desarrollo de habilidades sociales como la autonomía.
Las investigaciones realizadas, dependientes de recursos financieros aportados por los mismos laboratorios, están fundamentalmente orientadas a identificar las presuntas causas biológicas del problema y su respuesta a tratamientos farmacológicos, acompañadas de un significativo desinterés por indagar variables de tipo psicológicas, ambientales o sociales y los efectos de intervenciones pedagógicas o psicoterapéuticas.
Los métodos experimentales utilizados y los resultados obtenidos fueron tomados como objeto de importantes debates que provocaron encendidas controversias por parte de los adeptos al trabajo mientras que, en los últimos años, las mismas se hicieron extensivas al público en general generando problemas que aun hoy no fueron resueltos.
El déficit atencional- y la ausencia de problemas de carácter emocional- son el común denominador del tipo de acercamiento propuesto por todas estas investigaciones que, de este modo, permiten identificar una patología de la atención y del movimiento que inhibe el aprendizaje, pero que, finalmente, sólo representa una rotulación que termina generando consecuencias dramáticas en el plano personal, familiar, escolástico y social.
Los que adhieren al presente documento, expertos de gran importancia en el ámbito clínico y experimental, esperan, con honestidad intelectual y profesional, llevar a la luz los más recientes y menos conocidos resultados experimentales científicamente acreditados, con el objetivo de aportar elementos para un debate académico que clarifique un tema de primer plano en el panorama del derecho a la salud de los menores que compete a la comunidad científica y a la sociedad civil en general , propiciando para esto, la utilización de recursos metodológicos de investigación que permitan una aproximación éticamente más correcta en las confrontaciones de un problema que hace ya medio siglo es víctima de una metodología de aproximación inservible.
El ADHD como enfermedad
Es ya costumbre definir al ADHD, en primer instancia, como una "enfermedad genéticamente determinada", considerando las condiciones psico sociales de los sujetos como variables ajenas a la etiología del problema que presentan o entendiendo a las mismas, en términos de "conmorvilidad" , es decir, en calidad de trastornos secundarios generados a consecuencia del trastorno genético que padecen, influidos a su vez, por el tiempo transcurrido sin atención y la gravedad con que se presenta la sintomatología.
A propósito, resulta necesario aclarar que aun no hay evidencias experimentales que demuestren la causa directa a través de algún gen o pool de genes y que, ninguno marcador biológico (fenotipo) pudo hasta el momento ser identificado con certeza.
Las investigaciones realizadas con gemelos monocigóticos, bicigóticos y hermanos, parecen fuertemente viciados por la no demostrada presunción que el ambiente en el cual los chicos crecen sea siempre igual. Es virtualmente imposible que esto suceda. Más aún si se considera además que, las mencionadas investigaciones tampoco han tenido en cuenta el hecho, científicamente demostrado, que los genes que dirigen la síntesis de las proteínas son a su vez influenciados por factores ambientales tales como el stress, los traumas, la carencia de sensibilidad por parte de los padres, etc.
La presencia en los árboles genealógicos de este tipo de perturbaciones tampoco representa una prueba científica aceptable sobre la validez genética del ADHD, en cuanto no fueron tenidas en cuenta las variables "aprendizaje por imitación" y "aprendizaje por condicionamiento" y sus potentes efectos plasmados en las conductas, que hace casi un siglo fueron experimentalmente demostradas por la Teoría del Comportamiento y acerca de las cuales hoy no existen legítimas dudas.
Sobre este tema, en una entrevista realizada el 10 de octubre de 2002 por Fortline a F.X.Castellanos luego de la publicación de una meritoria y acreditada investigación de la Escuela de Psiquiatría Organista («F. Xavier Castellanos y otros, Developmental Trajectories of Brian Volume Abnormalities in Children and Adolescents With Attention- Déficit/Hyperactivity Disorder, Journal of the American Medical Association (JAMA 2002; 288:1740-1748)» ) resulta pertinente resaltar que, a la pregunta del entrevistador:
«¿Cuán próximos estamos a identificar un marcador biológico para el ADHD?» Castellanos responde:
«No lo sé, no pienso que lo sabremos hasta que no lo encontremos. Me gustaría encontrar un marcador biológico; me gustaría encontrar cualquier contestación objetiva, alguna cosa que nos dé la confirmación de cuanto hemos entendido sobre como funciona el ADHD. El problema es que indagamos en la oscuridad, y no sabemos a donde nos conducirá la investigación. Mi opinión personal es que andaremos a tientas por los próximos 3 o 4 añosÉ»
La tesis de la "enfermedad" entonces, sólo se trata hasta el momento de una mera "hipótesis" y la utilización de los términos "enfermedad" y "enfermedad mental" son hoy por hoy ilegítimos en el ámbito científico.
El ADHD por lo tanto, a lo sumo, sólo puede considerarse en términos hipotéticos como un simple grupo de comportamientos disfuncionales, demasiado ambiguos como para identificar una enfermedad.
La insuficiente definición de estos comportamientos-síntomas desde el punto de vista operacional, impide configurar netamente al ADHD como un cuadro psicopatológico con entidad propia.
Sobre las bases de los resultados científicos actualmente disponibles, el diagnóstico de ADHD corre el riesgo de ser sostenido fundamentalmente por motivaciones de carácter económico y no direccionado en función de un real beneficio del niño/paciente.
El diagnóstico de ADHD
En función de lo expuesto en relación al concepto de enfermedad, el diagnóstico utilizado resulta evidentemente insuficiente. El manual diagnóstico de la APA resalta en el DSM-IV que:
«Éno hay test de laboratorios que permitan confirmar el diagnóstico »del «Trastorno de déficit de atención con/sin hiperactividad»
En el documento «2000 American Academy of Pediatrics Annual Meeting Attention Deficit Hyperactivity Disorder: Current Diagnosis and Treatment», Mark L. Wolraich, MD responde:
«El diagnóstico del ADHD se relaciona con criterios limitados. El diagnóstico depende de la observación del comportamiento de los chicos por parte de diversas fuentes, en particular padres y docentes, con frecuencia con grandes desacuerdos entre ellos y sin un método claro para resolver estas discrepancias. Una de las fuentes de discrepancias es el hecho que los comportamientos son influenciados por el ambiente. La clase escolar podría dar lugar a comportamientos distintos de los observados en casa. Además, resulta imposible realizar un análisis comparativo entre las diversas observaciones recogidas dado que las mismas son absolutamente subjetivas a causa de la ausencia de criterios específicos que tornen competente la observación de los comportamientos; los observadores deben utilizar su criterio personal para juzgar.
Además, los comportamientos a ser observados y evaluados son los mismos independientemente de la edad y el estado de desarrollo del niño, desconociéndose que en realidad el mismo varía en función del crecimiento".
Si analizamos con atención los comentarios de los test experimentales que los especialistas utilizan para determinar el umbral de atención y de hiperactividad, emergen datos que nos inducen a reconsiderar muchas de nuestras pautas convencionales. Surge por ejemplo que, resulta esperable que los niños estén dispuestos a prestar atención a tareas aunque éstas no sean relevantes para el aprendizaje y ni siquiera sean de su agrado.
Se habla por lo tanto, de "carencia de atención en un contexto de baja motivación ", o, peor aún, de "comportamientos hiperactivos" en entornos familiares sumamente patógenos.
Parece al menos discutible que todo esto pueda ser transformado en una enfermedad de carácter biológico, mientras resulta evidente que, están implicadas dinámicas personales y sociales de variada naturaleza que no han sido consideradas hasta hoy por las mencionadas investigaciones científicas.
Frente a los disturbios de la atención y de la hiperactividad, sería necesario efectuar una seria investigación médica estándar y un profundo análisis de las relaciones familiares y sociales de los pequeños pacientes, de sus condiciones, estilos y niveles de aprendizaje real (lo que involucra también a la escuela) y de otros factores que pueden ser la causa de los comportamientos anormales del niño.
Se debe concluir por lo tanto que este "trastorno" no cuenta aún con una legitimación científica tal como para permitir un diagnóstico cierto que no deje lugar a dudas medianamente razonables.
La terapia farmacológica y sus efectos
La curación es un procedimiento terapéutico que, removiendo las causas que han generado la patología, lleva al reestablecimiento del estado de salud.
El alivio y la remisión de los síntomas, aunque constituyan eventos importantes, no permiten calificar una intervención terapéutica como curación.
Tanto la curación como los tratamientos sintomáticos deben garantizar el respeto de la dignidad humana y la integridad psicofísica, condiciones que la mayor parte de los psicofármacos actualmente en comercio no reúnen. No hay dudas acerca de que tales productos farmacéuticos tienen efectos colaterales graves, incluidos la muerte del paciente.
La supresión de los síntomas se manifiesta mientras el paciente consume regularmente el fármaco. Ni bien se produce la interrupción del mismo resurge nuevamente la situación anterior al período en que se realizó de manera regular el tratamiento farmacológico. Es este el motivo por el cual se hace necesario el suministro a largo plazo además que, su interrupción es desaconsejada por los propios especialistas y la misma industria farmacológica.
En un documento de Diciembre de 1999 «Long-Term Effects of Stimulant Medications on the Brain» el NIMH (Instituto Nacional de Salud Mental), declara que:
«Los estimulantes suprimen los síntomas del ADHD pero no resuelven el desorden y, como resultado, los chicos etiquetados como ADHD son tratados con estimulantes por muchos añosÉ»
«La terapia con estos productos farmacéuticos no mejoran el rendimiento escolar de los niños, en tanto que los procedimientos vinculados con el aprendizaje suponen algo mucho más complejo que el simple "prestar atención"». Afirma el Profesor Cesare Cornoldi, de la cátedra de Psicología de la Universidad de Padova, en referencia a la prescripción de Metilfendiato.
«Esta bien recordar que se pueden registrar efectos positivos en el control de la impulsividad, de la hiperactividad y de la atención mientras dure el suministro de fármacos pero los disturbios del aprendizaje, de la conducta y de la interacción social necesitan de intervenciones de naturaleza diferente. Generalmente la terapia farmacológica resulta crónica porque se realiza de manera excluyente; los intentos de evaluar qué sucede con la suspensión del fármaco se realizan en ausencia de otro tipo de intervenciones terapéuticas ya sean estas psicológicas o pedagógicas motivo por el cual los niños en breve, vuelven a presentar los mismos síntomas».(Cesar Cornoldi, "Hiperactividad y Autorregulación Cognitiva », Erickson, 2001, Pág.188.) ..
En 1993 el departamento de Educación de los Estados Unidos encargó a James. M Swanson, Director del Centro de Estudios sobre ADHD de la Universidad de California, Irving (UCI), notable sostenedor de la tesis biológica de ADHD y en todo de acuerdo con el uso de psicofármacos en menores, que se haga cargo de la conducción de una investigación centrada en la meritoria eficacia de la Ritalina
Fueron consultadas 300 revistas (9.000 artículos) navegando en 55 años de literatura.

Se puede concluir entonces, que los fármacos no mejoran el aprendizaje escolar, ni curan la presunta patología ADHD. Sólo actúan sobre los síntomas permitiendo una mejor aceptación social de los niños por parte de los adultos.
Poca atención se prestó a estudiar la repercusión psicopatológica que los tratamientos farmacológicos tienen sobre los niños y sobre las consecuencias del uso de estimulantes que se comercializan como una droga "novedadosa" mientras, lamentablemente, se banalizan sus potenciales efectos colaterales a mediano y largo plazo tristemente conocidos en el pasado.
Los casos que necesitan atención bajo el perfil clínico constituyen una minoría que deberían ser prioritariamente tratados con instrumentos de carácter pedagógico (pedagogía tradicional y clínica), mientras que, en su lugar sin embargo, la realidad es que en toda Italia se están utilizando este tipo de instrumentos pero bajo la forma de protocolos estándar que sólo sirven para codificar a los niños y/ o realizar intervenciones del tipo de la que proponen los instructivos de adiestramiento conductual.
Reinterpretar los datos
En los últimos años se han dado ha conocer numerosas investigaciones que identifican correlaciones de diferentes naturaleza con el ADHD. Se tratan de patologías físicas, reacciones a terapias medicamentosas, condicionamientos ambientales de varios tipos, embarazos desfavorables, trastornos psicopatológicos de diverso grado y naturaleza pero que, por presentar una sintomatología semejante o compatible con la que se describe como ADHD obtienen, en función de este criterio, el mismo diagnóstico.
La "nosografia" ADHD genera de hecho gran confusión despistando a aquellos médicos que omiten realizar una investigación exhaustiva del conjunto de factores etiológicos intervinientes, con un daño potencialmente importante para la salud de los menores.
No podemos olvidar que, estudiar, estar quietos y atentos en la escuela exige de cualquier niño un sacrificio que será resuelto de diferente manera en armonía con la Curva de Gauss y que, las variables que pueden explicar tales variaciones son tan numerosas que, por ahora, no estamos en condiciones de realizar una evaluación acabada de las mismas ni de expresar juicios clínicos concluyentes.
Todas las correlaciones que pueden observarse asociadas ¿pueden ser interpretadas como causas?.
¿Podemos considerar como hipótesis que la sintomatología ADHD sea en realidad una constelación inespecífica de síntomas, indicadores de un desequilibrio de la persona, que remite a factores causales de la más variada naturaleza?
¿Podemos abolir la nosografia ADHD con la carga ideológica que presupone así como años atrás se hacia con la homosexualidad (originalmente considerada como una enfermedad mental de la misma manera que se hace hoy con el ADHD)?
Este es el verdadero desafío que tenemos en frente, una hipótesis que merece toda la atención científica que seamos capaces de aportar, una forma diferente de hacer experimentos y un aproximación éticamente distinta de la que propone la utilización de psicofármacos con niños y adolescentes, que sólo se deberían indicar en casos excepcionales, con máxima cautela y como último recurso en situaciones extremas, a fin de prevenir y contener los posibles riesgos de abusos a mediano y largo plazo, como aquellos que, en no pocas ocasiones aparecen documentadas en la literatura científica que se desprende de fuentes confiables de información.

Somos un grupo de Psicólogos psicoterapéutas que trabajamos desde hace años en los Centros de Rehabilitación acreditados antes el Sistema Sanitario Nacional italiano y los Centros Sanitarios Locales (AASSLL) de la región Campania, cuya administración local ha remitido una carta circular que invita a los distritos sanitarios de la ASL Na 1 (centro sanitario local de la ciudad de Nápoles) a que cierren todo tipo de terapia psicoterápica, que se realizaba en colaboración con los susodichos Centros de Rehabilitación, antes del 31 diciembre de 2006. Esta circular perjudica el derecho de 4200 pacientes al tratamiento psicoterápico, además del puesto de 300 Psicólogos psicoterapéutas. La psicoterapia es un tratamiento imprescindible para las personas que la necesiten. Por tanto el Sistema Sanitario Nacional no puede prescindir de nosotros, a menos que esté dispuesto a sufrir una gran pérdida. |
